Bio:
The Jedi did with Zett Jukassa did what Obi-Wan and Yoda should've done with the baby Luke Skywalker: they changed his name to protect him from his family's dubious past. He only learned of his real name, Warpoc Skamini, when it became clear to the Jedi Council that his Force-connection to the murder scene of his parents might help reveal their killers.

His Jedi Master, Mierme Unill, thus helped Jukassa unlock his brief pre-Jedi past.

Following the resolution of the case--which revealed that the Black Sun had killed his parents--Jukassa went by both his birth and Jedi names.

At the time of Attack of the Clones--which was relatively soon after the murder investigation--Jukassa seemed to be preferring the name Warpoc, at least within the confines of the Jedi Temple. Around his death at the hands of the clones directly commanded by Darth Vader, he was seen to be using the name "Jukassa" again.

Notes:
In Episode II, Jukassa is the Jedi Padawan that walks up to Jocasta Nu to ask a question in the Jedi Archives after Obi-Wan Kenobi speaks to her. In Episode III, he is the child that, shortly after Order 66 is implemented, attacks and kills several clone troopers before being shot and killed. His death is witnessed by Senator Bail Organa. His first name is a play on the name of the actor who portrayed him: George Lucas' son, Jett Lucas. His first appearance is assumed to be Episode II adaptation, but this is currently unconfirmed. He must certainly appear in the Episode III adaptation, however.
